What is Diflucan?

Diflucan is an azole antifungal which belongs to the group of imidazoles and triazoles as a derivative of triazole. The active ingredient is used in the treatment of infections with fungal pathogens for humans.

Pharmacological effects

Depending on the dose, the active ingredient may also have a fungicidal effect (fungicide). After application, the active ingredient enters the bloodstream either through the gastrointestinal tract (including oral ingestion of capsules) or directly (injection administered intravenously) and is distributed throughout the body. Unlike the human cell membrane, which includes, among other things, cholesterol, ergosterol is the most important component of the yeast cell membrane.

In the cytochrome P450 system, Diflucan inhibits an important enzyme for the synthesis of ergosterol, 14-alpha-demethylase, which blocks the conversion of lanosterol to ergosterol. Blocked-modified building materials cause defects in the yeast cell membrane and alter certain metabolic processes that control the division of fungal cells.

Pathogens can no longer multiply (fungistatic effect). However, fluconazole has a significantly weaker inhibitory effect on demethylase in the human body.

Medical application and use

Diflucan is used in particular for systemic (internal) therapy of yeast infections of the genus Candida (known as cadidoses), of which Candida albicans is the most important representative.

As a rule, only the skin and / or nails (infections of the nails and athlete's feet) or the mucous membranes (oral thrush, vaginal fungus) are affected by candidiasis. In immunocompromised people, a fungal infection can also, in rare cases, affect the internal organs. As a result, fluconazole can also be used prophylactically to prevent fungal infection in people with weakened immune systems as a result of chemotherapy and / or radiation therapy.

In addition, Diflucan has been shown to treat meningitis (inflammation of the meninges) caused by infections with the yeast fungus Cryptococcus neoformans. Fluconazole can also be used prophylactically in people living with HIV who have an increased prevalence for this specific fungal infection (opportunistic infection). If topical or topical application of other antifungal agents is not successful in vaginal candidiasis, Diflucan can also be used as part of systemic therapy.

Risks and side effects

Diflucan is characterized as a relatively newer representative of triazoles by far fewer side effects and interactions with older active substances in this group. However, treatment with fluconazole is not completely safe and may be correlated with various side effects.

Diflucan therapy is often associated with nausea, vomiting, and gastrointestinal disturbances such as abdominal pain and diarrhea. Increased levels of enzymes for alkaline phosphatase and aminotransferases are also common. In some cases, loss of appetite, indigestion such as constipation or flatulence, dizziness, headache, cramps, increased sweating, sensitivity disturbances such as tingling, liver problems, jaundice , anemia and weakness and fever can be diagnosed.

Very rarely, among others, angioedema, cirrhosis, tissue necrosis and Stevens-Johnson syndrome may be associated with treatment with fluconazole. Diflucan is contraindicated in case of hypersensitivity to the active substance or to other antifungal agents, to pronounced hepatic function and to cardiac arrhythmias as well as impaired cardiac function.
