The active ingredient clindamycin is an antibiotic belonging to the pharmacological category of lincosamides. Cleocin is a so-called semi-synthetic derivative of the substance lincomycin.
What is Cleocin?
Clindamycin belongs to the sub-group of lincosamide antibiotics. The active ingredient is obtained from lincomycin and is then available in chlorine form. At higher doses, however, the drug has a bactericidal effect, so it kills bacteria.
The active ingredient clindamycin is primarily effective against aerobic Gram-positive agents. They are, for example, staphylococci or streptococci. In addition, the substance Cleocin also has an effect against anaerobes, in particular against Gram-negative germs and chlamydia. If the substance clindamycin is used with the substance pyrimethamine, the compound also acts against Toxoplasma gondii.
Pharmacological effects
The substance Cleocin inhibits protein synthesis in bacteria. The substance acts similarly to macrolides which bind to a specific subunit of the ribosomes. As the mechanism of action is the same, cross resistance occurs in some cases.
When the drug clindamycin is administered orally, approximately 90% of the active ingredient is absorbed. At the same time, 92 to 94% of proteins are linked to plasma proteins. Basically, the plasma half-life of the substance Cleocin is approximately 2.4 hours. The substance enters the internal organs as well as the bone marrow, pleural fluid, skin and breast milk. The drug is also able to penetrate the placental barrier. The active ingredient is converted in the liver. Finally, the drug clindamycin is excreted in the urine and stool.
Inhibiting bacterial protein synthesis stops an enzyme called peptidyl transferase. The spectrum of action of the drug Cleocin is relatively wide. For example, it works against Gram-positive cocci, such as Streptococcus pyogenes or pneumoniae. Regarding anaerobes, the drug is effective against Actinomyces, Peptostreptococcus, Fusobacterium and Bacteroides, for example.
The antibiotic clindamycin, on the other hand, has little or no effect on aerobic Gram-negative rod-shaped bacteria, enterococci, types of Haemophilus or Neisseries.

Medical application and use
The drug Cleocin is used for various infections. Clindamycin is often used in particular in the treatment of inflammation of lung abscesses, chronic osteomyelitis, diabetic foot osteomyelitis or infections of the oral cavity.
In addition, the drug is used for infections of the joints and bones or female genitals. Cleocin can also be used to treat acne vulgaris.
The possible fields of application are similar to those of other well tolerated macrolides. Staphylococcal infections are an exception. Because macrolides generally have no effect against them. In addition, clindamycin is also used as a substitute for patients allergic to penicillin.
In veterinary medicine, Cleocin is also used for serious infections of the skin, eyes or respiratory tract. Basically, clindamycin is a reserve antibiotic, so it is not always used as the first choice for possible indications.

Risks and side effects
In the context of therapy with the drug clindamycin, a number of undesirable side effects are possible, which vary depending on the individual case. The medication mainly causes gastrointestinal symptoms such as diarrhea, vomiting or cramps in the stomach area.
In addition, inflammation of the pseudomembrane colon is possible, which in some cases results from taking antibiotics. This leads to complications that occur more frequently with Cleocin than with other types of antibiotics. In this case, treatment with the active substance should be stopped immediately. The rare side effects of the drug include itching, inflammation of the skin and vaginal catarrh.
Clindamycin is not indicated during breast-feeding because the active substance is excreted in breast milk. Other potential side effects include liver damage associated with an increased concentration of transaminases. At the same time, the risk of allergies and leukocytopenia increases.
In principle, the drug Cleocin should not be taken if the patient is allergic to the antibiotics lincosamide. In case of side effects or other complaints, a doctor should be consulted immediately.
