Pharmacological effects

The drug trimethoprim is mainly characterized by a bacteriostatic effect. Because the substance prevents the so-called dihydrofolate reductase, so the metabolism of folic acid in gram-negative and gram-positive bacteria is impaired.

If the substance is used alone and not in combination with other active ingredients, uncomplicated urinary tract infections or upper respiratory tract infections are often the cause.

Basically, the drug Bactrim affects the dihydrofolate reductase of bacteria. Dihydrofolic acid is converted to tetrahydrofolic acid. For bacteria cells, tetrahydrofolic acid plays an important role as a substrate, which is necessary for the formation of thymidine and purines. If these particular building blocks of DNA are not present, the growth of bacterial cells is impaired.

Trimethoprim generally has only minor effects on human dihydrofolate reductase during treatment. In principle, the spectrum of action of the substance Bactrim extends to a large number of Gram-positive and Gram-negative pathogens. However, it should be noted that treatment with trimethoprim alone quickly becomes resistant to antibiotics.

When taken orally, absorption of Bactrim is relatively rapid. The active ingredient reaches its highest concentration in the serum about two hours after administration. The half-life is approximately twelve hours. The drug trimethoprim is metabolized by the liver and then excreted by the kidneys. It is therefore important to note that if the kidneys are weak, the metabolites of the drug can accumulate.

The drug Bactrim is used relatively rarely as part of a monotherapy. Instead, it is mainly used in conjunction with the cotrimoxazole substance in the form of a so-called fixed combination, by which the active ingredient sulfamethoxazole is used. If trimethoprim is combined with the active ingredient sulfamethoxazole, the possible scope is also widened. The substance sulfamethoxazole belongs to the category of sulfonamides.

In combination with Bactrim, the treatment of Toxoplasma gondii or Pneumocystis jirovecii is possible. At the same time, however, the spectrum of potential side effects is widening.

The combination of the active ingredients trimethoprim and sulfamethoxazole is known as cotrimoxazole. This compound is also suitable, for example, for the treatment of autoimmune granulomatosis with polyangiitis.

Pneumocystis pneumonia is a particular indication. It is necessary to take cotrimoxazole in large doses over a long period.

As part of monotherapy, Bactrim can be administered, for example, for uncomplicated cystitis and for the prevention of recurrent urinary tract infections in children. Basically, monotherapy with the active ingredient trimethoprim is the subject of critical debate.

    As part of therapy with the drug Bactrim or combination products that contain this active ingredient, various side effects and unwanted complaints are possible. These occur with different frequencies and vary depending on the individual case.

    Usually, trimethoprim is a well-tolerated preparation. In some cases, however, gastrointestinal problems arise. Itchy rashes are also possible. In rare cases, the active ingredient inhibits the important dihydrofolate reductase in humans, resulting in a tetrahydrofolic acid deficiency. This disrupts hematopoiesis, which leads to anemia, thrombocytopenia, or leukopenia.

    In addition, some patients experience rashes that are sometimes associated with itching. Serious skin irritations or Lyell's syndrome are less common. Aseptic meningitis is also possible when taking Bactrim. Patients sometimes suffer from fever and there are disturbances in the image of the blood. Phototoxic reactions and inflammation of the pancreas occur very rarely.

    The drug trimethoprim interacts with certain substances and therefore should not be taken at the same time.
